Create a Facilities Service Request (FSR)

NOTE: Do not file an FSR without going through the appropriate channels first.

The solution to this problem is to contact Myron myronm@tft.ucla.edu or the appropriate facilities coordinator to file an FSR on your behalf with campus facilities management to perform maintenance on the affected facilities.
